What You'll Do
As a Case Manager, you'll be a trusted advocate and partner in each individual's recovery journey. You'll help clients identify their goals, overcome barriers, and access the services and support they need to thrive.
- Build meaningful relationships with individuals and families through compassionate, respectful, recovery-focused support.
- Empower individuals to reach their goals by conducting strengths-based assessments and developing personalized service plans.
- Coordinate care and services with behavioral health providers, medical professionals, hospitals, families, and community partners.
- Connect clients with resources for housing, healthcare, employment, transportation, financial assistance, and other essential needs.
- Advocate for individuals when working with healthcare providers, courts, government agencies, and community organizations.
- Support clients through challenges and crises by coordinating appropriate services and helping them navigate difficult situations.
- Monitor progress and celebrate growth by evaluating individual needs and adjusting service plans to support continued recovery.
- Maintain accurate documentation that meets agency, Medicaid, and regulatory requirements.
- Collaborate and continue learning through team meetings, clinical supervision, training, and professional development.
- Work in the community by meeting clients where they are and, when appropriate, providing transportation to appointments and services.

What We're Looking For
- Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related field (or equivalent education and experience in the human services field).
- ·Valid driver's license, reliable transportation, and current automobile insurance.
- ·Strong interpersonal, communication, organizational, and time management skills.
- ·Ability to work independently while collaborating effectively within a multidisciplinary team.
- ·Basic computer proficiency and the ability to use electronic health records and other required technology.
Preferred Qualifications
- Experience providing case management or care coordination in behavioral health or human services.
- Knowledge of severe and persistent mental illness, recovery-oriented services, trauma-informed care, and evidence-based practices.
- Familiarity with Medicaid requirements and community behavioral health resources.
- Understanding of regulations and best practices related to community mental health services.
