Position summary
Carry a caseload of participants from enrollment through at least one year of follow-up after completion. The Case Manager builds each participant's Individual Employment Plan, tracks progress through training and into work, and administers Barrier Reduction services so that practical obstacles do not end a participant's training. About 60% of the role is assessment, planning and tracking, and 40% is needs assessment and supportive services.
Essential duties and responsibilities
• Complete a needs assessment with each participant at enrollment and update it throughout the program.
• Develop the Individual Employment Plan with the participant within 10 business days of enrollment and amend it at each transition.
• Meet each participant weekly during training, track attendance and progress with instructors, and respond the same day to problems that interrupt training.
• Recommend Barrier Reduction services under the Hub's policy, such as transportation, child care, work clothing and emergency assistance, and keep the required documentation.
• Connect participants with community resources and coordinate referrals and handoffs with partner providers.
• Work with transition services staff on job readiness and placement, and provide retention support after placement.
• Carry out monthly follow-up contact for at least one year after completion.
• Record services, contacts and outcomes in the CEJA Reporting System within five business days.
Qualifications and experience
• Bachelor's degree in social work, human services, counseling or a related field preferred; equivalent experience may be considered.
• Two or more years of case management or career coaching experience with adults who face barriers to employment.
• Experience with people returning from incarceration, low-income households or English language learners.
• Knowledge of community resources in the service area.
• Accurate record keeping and sound judgment with confidential information.
Training and authorization required
• Department authorization for participant service work and CEJA Reporting System training before working with participants.
