Responsibilities

• Provide occupational therapy services through a telehealth platform.
• Complete evaluations, reassessments, and treatment documentation as appropriate.
• Develop and implement individualized treatment plans based on patient needs.
• Collaborate with facility staff and interdisciplinary care teams.
• Monitor patient progress and adjust interventions as needed.
• Maintain accurate documentation and comply with applicable clinical and regulatory requirements.

Qualifications

• Active Occupational Therapist license in Michigan (required).
• Previous skilled nursing facility (SNF) experience required.
• Prior telehealth occupational therapy experience required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating effectively with facility teams.
• Proficiency with electronic medical records and telehealth technology.
