About Wellness Equity Alliance

Wellness Equity Alliance is a national multidisciplinary health organization that designs and delivers integrated, community-based care for populations most impacted by health inequities. We do this through mobile and field-based models, providing medical care, behavioral health services, substance use treatment, harm reduction, and care coordination in nontraditional settings such as encampments, schools, reentry sites, and rural communities, as well as with sovereign tribal nations. Grounded in trauma-informed, culturally responsive, and data-driven practices, WEA combines clinical expertise, lived experience, and advanced population health analytics to reduce barriers to care, improve continuity, and strengthen local systems. We have partnered with more than 60 public agencies, managed care plans, and community-based organizations across the U.S. to implement scalable, sustainable programs that are advancing health equity and improving outcomes for historically marginalized populations.

We are known as Renegades, Rebels, Disruptors, and Dreamers. If that sounds like you, we want you on our team.

Position Summary

The Care Coordination & Housing Navigation Supervisor leads the frontline teams who are often a person's first point of contact with WEA, whether by phone or in the field. Reporting to the CHAC Operations Lead, this role supervises Patient Care Coordinators in the Community Health Access Call Center and Housing Navigators in the Housing Navigation Program.

The Supervisor ensures every member we reach, from a hesitant caller to an unhoused member navigating the housing system, is met with respect, follow-through, and a clear path to services, including CalAIM Enhanced Care Management (ECM) and Community Supports.

Key Responsibilities

Team Leadership and Supervision

  • Supervise, coach, and support Patient Care Coordinators and Housing Navigators, including hiring, onboarding, training, scheduling, timekeeping approval, and performance management.
  • Lead regular team meetings, case conferences, and quality assurance and coaching sessions, including call monitoring and documentation review.
  • Model and reinforce trauma-informed care, harm reduction, motivational interviewing, and de-escalation practices across both teams.
  • Serve as the escalation point for complex member situations, coordinating with Care Managers, the Street Medicine team, and clinical leadership.
  • Partner with the CHAC Operations Lead on staffing plans, coverage, and workload distribution.

Community Health Access Call Center

  • Oversee high-volume telephonic outreach to prospective CalAIM members identified through health plan referrals, community referrals, and internal lead lists.
  • Set and monitor productivity standards for call volume, successful contacts, and enrollment conversion, and coach staff to meet them.
  • Ensure accurate eligibility screening, scheduling of enrollment appointments, and timely warm handoffs to Care Managers.
  • Maintain and improve outreach workflows, call scripts, and disposition tracking.

Housing Navigation Program

  • Oversee Housing Community Supports navigation for Medi-Cal members experiencing homelessness or housing instability, including shelter placement, housing applications, and housing readiness documentation.
  • Ensure timely completion of Coordinated Entry System (CES) assessments in HMIS.
  • Build and maintain relationships with housing and shelter providers, recuperative care, recovery programs, and other community partners.
  • Support tenancy education and stabilization for members who secure housing, and keep the Street Medicine team current on resources serving unhoused communities.

Data, Quality, and Compliance

  • Track and report program performance, including outreach outcomes, enrollments, housing placements, and follow-up timeliness.
  • Ensure all documentation meets HIPAA, CalAIM, and health plan contractual standards; participate in audits and quality improvement initiatives.
  • Identify barriers and recommend process improvements to the CHAC Operations Lead and program leadership.
  • Perform other duties as assigned.
Requirements

Required

  • High school diploma or GED.
  • Three (3) or more years of experience in care coordination, case management, housing navigation, patient access, or community outreach.
  • One (1) or more years of experience supervising or leading staff, including scheduling, coaching, and performance feedback.
  • Working knowledge of Medi-Cal and CalAIM programs, including Enhanced Care Management (ECM) and Community Supports.
  • Experience serving people experiencing homelessness, behavioral health conditions, or substance use disorders.
  • Demonstrated use of trauma-informed care, harm reduction, motivational interviewing, and de-escalation.
  • Experience using productivity or performance metrics to manage a team.
  • Proficiency with EHR or case management systems and Microsoft 365 or Google Workspace.
  • Valid California driver's license, reliable transportation, and current auto insurance.

Preferred

  • Bachelor's degree in social work, public health, human services, or a related field.
  • Experience with the Coordinated Entry System (CES) and HMIS.
  • Experience supervising call center or telephonic outreach teams.
  • Lived experience with homelessness, recovery, or the systems WEA's members navigate.
  • Bilingual in English and Spanish.

 Work Environment and Physical Requirements

  • Hybrid schedule combining remote work, at least 2 days in-office call center supervision, and field visits to shelters, housing providers, and community partner sites across Southern California.
  • Field settings may include encampments and other nontraditional environments with exposure to weather and unpredictable conditions.
  • Regular use of a computer and phone for extended periods; ability to sit, stand, and walk for portions of the workday.
  • Ability to lift up to 25 pounds occasionally.
  • Local travel by personal vehicle is required; mileage is reimbursed per WEA policy.
