At NADAP, our Health Homes care managers help about 1,800 New Yorkers manage chronic health conditions, mental health needs, and recovery, and connect them to housing, benefits, and the support they need to thrive. We're looking for an Assistant Vice President of Case Management to guide our team of about 70 dedicated professionals and help us grow our impact across New York City and Long Island.

What you will do

• Lead a passionate team of directors, supervisors, and care managers who show up for members every day.
• Shape the strategy and day-to-day operations of our NYS DOH Health Homes program.
• Build strong partnerships with the Lead Health Homes, health plans, hospitals, and community organizations we work alongside.
• Use data to celebrate wins, spot gaps, and keep service quality high.
• Mentor and develop staff and build a workplace where people want to stay and grow.

What you will bring

• A master's degree in social work, public health, or a related field 
• 7+ years progressively responsible roles in care or case management, with at least 4 years leading programs .
• Hands-on experience with the NYS Health Homes Program and Medicaid.
• A collaborative, steady leadership style and a genuine commitment to the people we serve.
