Concept Rehab is seeking a PRN Speech-Language Pathologist to join the rehabilitation team at our Columbus, Ohio, skilled nursing facility. What You'll Do

• Evaluate residents' speech, language, cognitive-communication, and swallowing needs.
• Develop and implement individualized treatment plans based on clinical findings and resident goals.
• Provide therapeutic interventions to improve functional communication and swallowing safety.
• Collaborate with nursing staff, physicians, and other rehabilitation professionals to coordinate resident care.
• Document evaluations, treatment sessions, and progress in accordance with applicable clinical and regulatory requirements.

What You'll Do

• Evaluate and treat residents with communication, cognitive, and swallowing disorders
• Develop individualized treatment plans that support functional outcomes
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Participate in care planning and discharge planning
• Educate patients, families, and facility staff
• Complete timely and accurate clinical documentation

Qualifications

• Master's Degree in Speech-Language Pathology
• Current Ohio Speech-Language Pathology license
• ASHA Certificate of Clinical Competence (CCC-SLP)
