Description

About the Role
Ensemble CIN is looking for a Care Coordinator I to help patients move safely and confidently from one point of care to the next. You’ll be the steady voice on the phone, the person who makes sure discharge instructions make sense, follow-up appointments happen, and nobody slips through the cracks. If you like organizing the details so patients and families can focus on healing, this role is for you.

  • Make sure patients understand their discharge instructions and complete their follow-up appointments
  • Connect patients to community resources that support their recovery
  • Help with scheduling and service verification for post-discharge care
  • Watch alerts on remote patient monitoring platforms and pass concerns to clinical staff right away
  • Escalate clinical or complex needs to licensed team members
  • Keep care coordination tracking current using our tools and systems
  • Document every communication accurately in the electronic medical record
  • Work alongside physicians, providers, and community agencies to keep care connected
  • Share ideas that make our workflows smoother and our patients’ experience better

  • Scheduling experience preferred
  • One year of experience in a healthcare or customer service setting preferred
  • Medical Assistant certification preferred
  • Bilingual Spanish-English communication skills preferred
  • Comfort with electronic medical records and everyday communication platforms
  • Working knowledge of medical terminology, confidentiality, and HIPAA
