Your Role in Home Health

As a Physical Therapist (PT) with VRNS, you’ll deliver skilled physical therapy services in patients’ homes. You’ll assess mobility and functional needs, establish individualized treatment plans, and help patients improve strength, balance, movement, and independence. Home care gives you the opportunity to build meaningful patient relationships while tailoring treatment to each person's goals and environment.

Key Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop individualized treatment plans based on patient needs and goals
• Provide therapeutic exercises and interventions to improve strength, mobility, balance, and function
• Help patients improve transfers, gait, and safe mobility
• Assess home environments and recommend appropriate safety strategies or equipment
• Educate patients and caregivers on exercises and techniques
• Monitor progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, occupational therapists, and other healthcare professionals
• Complete accurate and timely electronic documentation
