About Post Acute Analytics
Post Acute Analytics (PAA) is a healthcare technology company building the operating system for post-acute care. Millions of Americans transition each year out of the hospital into skilled nursing, home health, or back home, and those transitions too often fail them. Readmissions, medication errors, missed follow-ups, families left in the dark. Our clinical intelligence platform, Anna, gives providers, payers, and caregivers the visibility and workflow tools to get the right care to the right person at the right time.
PAA combines deep clinical expertise, connected technology, and industry-leading datasets to help our partners deliver better outcomes, streamline care coordination, and improve the patient experience across the post-acute continuum. Today Anna operates across millions of covered lives, more than 40 states, and many of the industry’s most recognized payer and provider organizations.
What distinguishes PAA is how we work. Clinical judgment leads. Technology amplifies clinician expertise rather than replacing it. Client trust is infrastructure, not a metric. Post-acute care is genuinely complex, and we build for that complexity honestly.
PAA currently partners with many of the top health systems, most national health plans, and leading national and regional post-acute providers. What makes PAA distinctive isn’t the funding or the growth curve — it’s how we work. Autonomy and ownership are non-negotiable. AI amplifies expertise, it doesn’t replace it. User trust is infrastructure, not a metric.
The opportunity
The PAC Engagement Specialist makes sure post-acute care (PAC) facilities are actively taking part in the Anna workflows our clients’ care teams depend on: uploading clinical documents, confirming head-in-bed, uploading signed NOMNCs, and responding to conversations. You’ll join the PAC Engagement (PES) team within Client Success, reporting to the PAC Engagement Lead, and own engagement for assigned clients and facilities across a large and rapidly growing network.
Day to day, you’ll use engagement dashboards and reports to find facilities with no active user and facilities below engagement goals, then work them through outbound calls (including cold calls), email campaigns, and user training. Regular phone outreach from your personal cell phone and keeping facility records current are a significant part of the role.
This full-time role combines analysis and training with hands-on outreach. You’ll work closely with Network Operations, Client Success Executives, and the Training team, and you’ll represent PAC users internally.
Defining success
Success in this role is measured by:
- Growth in active Anna users at assigned facilities.
- Improvement in document, head-in-bed, NOMNC, and conversation performance at assigned facilities.
- Outreach that measurably moves engagement metrics, supported by accurate, current facility records.
What you’ll own
Engagement monitoring and prioritization
- Monitor engagement for assigned clients on a daily and weekly cadence using engagement dashboards and reports, tracking documents, head-in-bed, signed NOMNCs, and conversation responses against goals.
- Build outreach lists each cycle: facilities with no active user, and facilities with users that are below goal.
Facility outreach and training
- Make outbound calls from your personal cell phone, including cold calls to facilities with no active user, to find the right contact and register them in Anna.
- Run email outreach to below-goal facilities, including templated and mass email campaigns.
- Onboard and train new PAC users on Anna workflows, and coordinate with the Training team on onboarding and consistent messaging.
Data quality, escalation, and reporting
- Validate engagement data (for example, confirming document uploads completed through integration), keep facility records and outreach notes current, and measure whether outreach moved the metrics.
- Escalate integration gaps to Network Operations and repeat non-compliance or client-side issues to Client Success Executives; capture PAC user feedback for Product.
- Prepare engagement reporting for leadership and clients, including scorecards, PAC facility snapshots, and monthly reporting, and contribute to network-wide communications such as newsletters.

- Proven experience reaching providers by phone and email, including cold outreach, and building working relationships that improve performance.
- Comfort making regular outbound and cold calls.
- Ability to manage a large facility list and prioritize from data; organized and consistent with record-keeping.
- Clear, patient communicator with facility staff at all levels.

- Bachelor’s degree required.
Preferred
