Help Young Adults Rebuild Their Future After a Behavioral Health Crisis
At Concord Counseling Services, we believe that a behavioral health crisis does not define a person's future. Through our innovative Renew Program, we help young adults ages 18-25 successfully transition from hospitalization back into their communities, strengthen their support systems, and build lives filled with purpose, connection, and hope.
We are seeking a compassionate, driven, and community-minded Renew Case Manager to join our team. In this role, you'll work directly with young adults who have experienced a first-time behavioral health hospitalization or have been identified as at-risk through our partnership with Star House. You'll provide individualized support, connect clients to critical resources, and help them navigate the challenges of recovery, independence, employment, education, housing, and community living.
This is more than a case management position. It is an opportunity to walk alongside young people during a pivotal moment in their lives and help them discover that recovery is possible.
What You'll Do
•Build meaningful relationships with young adults using recovery-oriented and trauma-informed approaches.
•Meet clients where they are, including in homes, schools, workplaces, hospitals, and throughout the community.
•Help clients identify personal goals and develop actionable plans to achieve them.
•Connect individuals with behavioral health treatment, psychiatric care, housing, employment resources, educational opportunities, and community supports.
•Provide crisis intervention and stabilization support when needed.
•Collaborate with hospitals, treatment providers, families, and community partners to ensure continuity of care.
•Advocate for clients and help remove barriers that interfere with recovery and long-term stability.
•Empower young adults to develop the skills and confidence needed for successful independent living.
Qualifications
•Bachelor's degree in Social Work, Counseling, Human Services, Psychology, or a related field required.
•Ohio licensure as an LSW, LPC, or other qualifying behavioral health credential preferred.
•Experience providing community-based behavioral health services, crisis intervention, intensive case management, or psychiatric support services preferred.
•Knowledge of community resources, behavioral health systems, and recovery-oriented practice.
•Valid driver's license, reliable transportation, and the ability to travel throughout the community.
