Case Manager II
Description

The overall responsibility of the Case Manager is to manage every aspect of the case while fostering strong relationships with Participant/Individuals in Philadelphia and surrounding counties. 

Requirements

  

Management of Participant/Individual Relationships:

  • Introductory meetings within 24 hours of person becoming a Participant/ Individual. 
  • Execute Participant/Individual consent form. 
  • Complete Direct Care Worker Assignment Sheet. 
  • Conduct monthly phone inquiries to discern any issues or concerns. 
  • Home visits at least every 90 days to adjust care plans. 
  • Attending Individual Support Plan meetings.
  • Ensuring that recertification of Participant/Individual’ Medicaid benefits do not lapse. 
  • Submit consumer services survey on an annual basis. 
  • Serving as liaison between the Participant/Individual and Service Coordinator regarding services changes/issues. (i.e., increase of hours, temporary holds, hospitalizations, etc.)

Supervision of Caregiver Staff (Home Health Aides, Certified Nursing Assistants and Personal Care Attendants):

  • Ensuring timeliness and dependability of caregivers on their respective case load.  
  • Policy and procedure coaching and enforcement; report need for disciplinary action if necessary. 
  • Scheduling and conducting Meet and Greets and in service visits in the field as needed. 
  • Other duties as assigned.
