Nautilus Health is searching for a contract RN/LVN in the Dallas/Ft. Worth area.  As a PRN contractor, you'll visit patients in their homes following discharge — carrying out care plans as ordered by our physicians and APPs.  The pay is $50 per visit.

We are seeking skilled and compassionate Registered Nurses (RN) and Licensed Vocational Nurses (LVN) who enjoy working independently and are committed to providing high-quality clinical care to patients recovering after hospitalization.

This PRN 1099 contractor role involves traveling to patient homes to provide skilled nursing services in accordance with physician and advanced practice provider (APP) care plans. The position requires strong clinical judgment, attention to detail, and the ability to work collaboratively with a multidisciplinary care team.


Key Responsibilities:

  • Conduct in-home nursing visits for patients recovering after hospitalization or surgery.
  • Perform patient assessments and monitor recovery progress, reporting changes in condition to the provider team.
  • Carry out physician and APP-ordered treatment plans and nursing interventions.
  • Provide post-surgical care and monitoring to ensure safe recovery at home.
  • Administer medications, injections, and IV therapies within the scope of licensure.
  • Perform central line care and IV antibiotic administration.
  • Provide wound care, including incision care, wound irrigation and drainage (I&D) care, dressing changes, and monitoring for infection.
  • Manage Wound VAC therapy and ensure proper function and patient understanding.
  • Provide JP drain care and monitoring.
  • Perform J-tube and G-tube care following surgical procedures.
  • Provide new ostomy education and appliance management to patients and caregivers.
  • Educate patients and families on medications, wound care, device management, and recovery expectations.
  • Collaborate with physicians, advanced practice providers, and other members of the healthcare team.
  • Communicate patient progress, complications, or concerns promptly to the care team.
  • Maintain accurate and timely clinical documentation of patient visits, assessments, treatments, and outcomes. 
  • Assess home environments for safety concerns and recommend appropriate modifications when necessary.
  • Follow infection control standards, clinical protocols, and applicable regulatory requirements.
  • Deliver patient care that promotes dignity, safety, and quality of life.


Requirements

Qualifications:

  • Active Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in Texas, Oklahoma, or Arizona.
  • Graduate of an accredited school of professional nursing.
  • Minimum 1–2 years of clinical nursing experience required.
  • Home health, surgical recovery, infusion therapy, or post-acute care experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Experience with wound care, IV therapy, and post-surgical care management preferred.
  • Ability to work independently in a home care environment.
  • Excellent communication skills with patients, families, and healthcare providers.
  • Strong organizational and time-management abilities.
  • Reliable transportation and willingness to travel to patient homes.
  • Ability to lift, move, and assist patients as needed.
  • Comfort working independently while maintaining close communication with the provider team.
