JOB TITLE: Certified Nursing Assistant

REPORTS TO: Personal Care Home Administrator

SUPERVISES: N/A   

INTERNAL CONTACTS: Residents, Staff members  

EXTERNAL CONTACTS: Family Members and/or Representatives, Visitors 


SUMMARY: Under the supervision of the Personal Care Home Administrator, the Certified Nursing Assistant (C.N.A.) provides direct care to residents in the facility, primarily for their activities of daily living, assistance with therapeutic activities program, restorative programs documentation as required by the facility.


ESSENTIAL DUTIES AND RESPONSIBILITIES This summarizes the major essential duties and responsibilities regularly performed and normally assigned to this position. Listed below are the duties in descending order of importance and are as clear and specific as possible. Other duties may be assigned. May be necessary to assist in the evacuation of residents during emergency situations.

  • Assists residents with ADL (feeding, bathing, ambulating etc..) as needed
  • Efficiently obtain vital signs for indicated residents in a timely manner and report results to charge nurse.
  • Assists Residents to dining room for meal service timely and is responsive and available to the Residents during dining service.
  • Assist residents in and out of bed and chair or wheelchair with the correct use of the transfer belt (gait belt). May require the assistance of another aide, depending on weight and physical abilities. May require the use of a mechanical lift to move the resident.
  • Performs incontinence care for assigned residents timely and ensures that all residents are maintained with dry skin to prevent skin break down.
  • Effectively communicates with charge nurse with ideas and suggestions to improve the overall care delivered for assigned Residents.
  • Assist residents in dressing or undressing. Fold or hang clean items and place soiled items in hamper (family laundry) or lined barrel (Meadows laundry).
  • Assist residents in using bathroom and incontinent care. May be required to provide, remove and clean bedpans, urinals and bedside commodes.
  • Take vital signs of residents, e.g. temperature, blood pressure, pulse, and respiratory rate and record information. Responsible to weigh residents as scheduled.
  • May be required to clean equipment such as bedpans, basins and empty catheter bags.
  • Remove soiled bedding and replace with clean linens as needed.
  • Assist in personal grooming of residents, e.g. brushing hair, teeth, shaving, trimming fingernails and toenails and general nail care. Monitor hygiene continuously and assist as needed to assure that each resident is clean and neat.
  • If resident is unable to reposition self, aide may be required to turn and reposition resident intermittently.
  • Assist residents to dining area for meals or provide meals in room, as resident prefers. May be required to assist in tray setup and feeding of residents and documenting intake of nourishment. Serve between meal nourishment and assist as needed. Record amount eaten and report any changes in residents eating habits. Except when contraindicated, maintain clean pitcher and cup, and fresh water at bedside of each resident.
  • Maintain each bedside unit in neat and orderly condition. Clean table surfaces as needed. Store personal items such as dentures, eyeglasses, combs, brushes in sanitary and convenient manner.
  • Make continuous observation of housekeeping and maintenance needs, e.g. broken fixtures, burned out light bulbs, soiled floors and curtains and reports needs promptly.
  • Maintain bedside care supplies, e.g. gloves, tissues, toothpaste; use charge system to record supplies obtained from Meadows stock. Notify charge nurse to have them call if items are to be supplied by family.
  • Maintain all service areas in clean and orderly condition.
  • Required to maintain resident documentation per Meadows policy.
  • Assist in assuring the maximum quality of life for all residents by maintaining privacy, accommodating individual preference, promoting independence, encouraging and assisting participation in facility activities and events and communication respectfully at all times.
  • Respond to all call lights promptly. Keep call light within reach of non-ambulatory resident when unattended.
  • Maintain continuous communication with charge nurses, supervisors and peers. Participate in report; communicate all changes and unusual observation concerning residents physical and psychosocial functioning.
  • Attend unit meetings as scheduled, all mandatory in-services and optional in-services per individual needs and interest, for a total of at least 12 hours annually. Outside educational offerings may be substituted for optional in-service hours by providing proof of attendance. Mandatory in-service cannot be substituted.
  • May serve on committees, as requested.


QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.


LANGUAGE SKILLS: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.


MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.


REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in, written, oral, diagram, or schedule form. 


*COMPUTER SKILLS: Proficiency with CareTracker Documentation system and basic computer skills. Familiar with telephone equipment and other various office equipment.  


*CERTIFICATES, LICENSES, REGISTRATIONS: State of Pennsylvania license as a Certified Nurse Assistant. Current CPR and First Aid is required.


OTHER SKILLS AND ABILITIES: Ability to effectively manage multiple priorities. Ability to work in a fast paced and quick changing environment.

While performing the duties of this job, the employee is required to talk, hear, stand, walk, sit, use hands to finger, handle or feel objects, and reach with hands and arms. 

The employee must be able to lift and/or move up to 50 pounds. The employee must be able to push up to approximately 200 pounds and be able to push such weight approximately 200 feet. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus.
