The TwelveStone Infusion Centers LLC, manages the Infusion needs of patient’s and maintains the continuum of care, from intake, assessment, and treatment through the completion of their visit. Our primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri, etc.). On occasion there is a need for sterile dressing changes and lab draws.
TwelveStone Health Partners is seeking a Part-Time Nurse Practitioner (NP) to join our outpatient infusion center in Cookeville, Tennessee. This Advanced Practice Provider (APP) position is responsible for delivering high-quality patient care, including patient assessment, treatment, chronic disease management, care coordination, patient education, and clinical oversight for individuals receiving infusion therapy.
We welcome Nurse Practitioners from a variety of clinical backgrounds who are passionate about patient-centered care and collaborative practice. Experience in infusion therapy, ambulatory care, specialty pharmacy, internal medicine, family practice, urgent care, neurology, rheumatology, gastroenterology, immunology, or oncology is preferred. Strong IV assessment and vascular access skills are highly desirable.
Essential Duties and Responsibilities
- Provide comprehensive patient assessment, treatment, and management for patients receiving infusion therapy in an outpatient infusion center setting.
- Deliver patient- and family-centered care while promoting positive clinical outcomes and patient satisfaction.
- Monitor patients before, during, and after infusion treatments and assist in the management of infusion-related reactions or clinical concerns.
- Facilitate coordinated care and communication among physicians, pharmacists, nurses, referral sources, patients, and caregivers.
- Educate patients and families regarding infusion therapies, biologic medications, treatment plans, and ongoing care needs.
- Serve as a patient advocate and assist patients in navigating the healthcare system.
- Collaborate effectively with multidisciplinary healthcare teams to support continuity of care and chronic disease management.
- Review, update, and recommend clinical policies, procedures, education, and competency standards that support quality patient care and regulatory compliance.
- Participate in quality improvement initiatives, patient service reviews, and program evaluations.
- Maintain accurate, timely, and compliant medical documentation and reporting.
- Perform occasional sterile dressing changes, laboratory specimen collection, and other clinical procedures as needed.
- Other duties as assigned.
Education & Experience
- Master's degree or higher in Nursing from an accredited Nurse Practitioner program required
- Current, unrestricted Nurse Practitioner license in Tennessee required
- National certification as a Nurse Practitioner required
- Current DEA registration and prescriptive authority required, or ability to obtain
- Minimum of two (2) years of clinical experience as a Nurse Practitioner preferred
- Experience in outpatient care, ambulatory care, infusion therapy, specialty pharmacy, primary care, family practice, internal medicine, urgent care, neurology, rheumatology, gastroenterology, immunology, oncology, or chronic disease management preferred
- Infusion therapy experience preferred but not required
- Strong IV assessment, vascular access, and patient education skills preferred
- Current BLS certification required; ACLS preferred
- Demonstrated ability to assess, treat, and manage patients with acute and chronic medical conditions in an outpatient setting
- Strong communication, collaboration, critical thinking, and clinical decision-making skills
- Ability to work independently while functioning effectively within a multidisciplinary healthcare team
