Our In-Home Infusion Services team brings specialized infusion therapy directly to patients, allowing them to receive treatment safely and comfortably in their homes.Our nurses play a critical role in delivering high-quality clinical care while building meaningful relationships with patients and caregivers. Through the administration of specialty and biologic medications, patient education, ongoing assessment, and care coordination, we help patients successfully manage complex medical conditions outside of traditional healthcare settings.


Position Summary


TwelveStone Health Partners is seeking a PRN Registered Nurse (RN) – In-Home Infusion to join our growing team in Kentucky. This is a field-based position providing infusion therapy services in patients' homes. Nurses travel within assigned service areas to deliver prescribed infusion treatments, perform patient assessments, provide education, and coordinate care with physicians and other healthcare professionals. The ideal candidate is a compassionate, self-motivated nurse who enjoys working independently, building meaningful patient relationships, and delivering exceptional patient-centered care.


Responsibilities

  • Safely administer infusion therapies, injectable medications, and other prescribed treatments in accordance with physician orders and clinical protocols
  • Conduct comprehensive patient assessments before, during, and after treatment
  • Monitor patient response to therapy and communicate significant findings or changes in condition to the prescribing provider
  • Review and interpret relevant patient information, including laboratory results and clinical assessments
  • Provide patient and caregiver education regarding disease states, treatment plans, medications, side effects, and self-care expectations
  • Coordinate care with physicians, pharmacists, case managers, and other members of the healthcare team to ensure continuity of care
  • Receive patient referrals and coordinate scheduling of home visits
  • Verify, implement, and document physician orders accurately and timely
  • Maintain complete and accurate documentation of patient assessments, interventions, treatment responses, and progress toward therapy goals in the electronic medical record
  • Serve as a patient advocate and assist patients in navigating their treatment journey
  • Deliver individualized, culturally competent care that respects each patient's dignity, autonomy, and preferences
  • Maintain confidentiality and compliance with HIPAA and company policies
  • Participate in quality improvement initiatives, clinical program reviews, and ongoing professional development activities
  • Facilitate clear and consistent communication among patients, caregivers, physicians, and interdisciplinary team members

Qualifications


Education & Licensure

  • Graduate of an accredited nursing program
  • Current, unrestricted Registered Nurse (RN) license in the state of Kentucky
  • Current BLS certification required
  • Valid driver's license and reliable transportation required

Experience

  • Minimum of one (1) year of nursing experience preferred
  • Previous infusion therapy, home health, ambulatory care, specialty pharmacy, oncology, critical care, emergency department, or medical-surgical nursing experience preferred
  • Experience caring for patients with chronic and complex disease states preferred
  • Strong peripheral IV insertion and vascular access skills preferred

  • Travel required within assigned service areas throughout Kentucky
  • Flexible PRN schedule based on patient needs and nurse availability
  • Mileage reimbursement provided in accordance with company policy
