The Xiente Teen Center provides vital youth development opportunities, operating from 3 PM to 9 PM to engage teens in productive and educational activities. Additionally, the center serves as a curfew center for youth from 9 PM to 2 AM, offering support and services to those brought in by the police after curfew hours.

About the Role

The Intensive Prevention Services (IPS) Case Manager supports at-risk youth in achieving social and academic success through comprehensive, intensive programming. The Case Manager helps prevent at-risk youth's entry or re-entry into the juvenile justice system. You'll work closely with an IPS Counselor as part of a small team, ensuring each youth gets tailored, consistent support.

The IPS program, funded by the City of Philadelphia Department of Human Services, is an after-school initiative providing early intervention services for youth exhibiting high-risk or at-risk behaviors.

What You'll Do

Direct Client & Family Support

• Conduct initial assessments with clients and families.
• Gather client/family information through interviews, case history, and observation.
• Make weekly home visits, often alongside the assigned Counselor.
• Assist with transportation of clients as needed.

Family & Community Engagement

• Link families to the program's parent education component and family activities.
• Consult with the IPS Counselor on referral needs — especially for substance abuse and mental health — and follow through on referrals.
• Collaborate with the IPS Counselor and team on step-down and discharge planning.

Case Planning & Documentation

• Collaborate with the team to build each youth's Individual Service Plan.
• Lead monthly case review meetings and track progress toward outcomes.
• Document all casework, including assessments, IPS records, and monthly statistical/narrative reports.
• Represent assigned youth at court appearances.
• Participate in interdisciplinary meetings and weekly supervision.
• Other duties as assigned.

Qualifications

Required

• Bachelor's degree in a relevant field (social work, behavioral health counseling, juvenile justice, education) — or equivalent experience working directly with youth.
• At least 2 years of experience working with youth or young adults.
• Strong interpersonal, verbal, and written communication skills.
• Proven ability to work collaboratively as part of a team.
• Proficiency with Microsoft Word, Excel, and Outlook.
• Strong organizational skills and comfort managing documentation and paperwork.
• Ability to secure PA Child Abuse, PA Criminal History, FBI, and National Sex Offenders clearances.
• Ability to secure health clearances every two years and a TB test once.

Preferred

• Bilingual (Spanish).
• Valid driver's license and insurance.
