JOB SUMMARY: This position includes a combination of care coordination, data management, and direct clinical duties for adults who have a history of substance use and who may still be actively using various substances. The Case Manager is responsible for screening individuals, doing a comprehensive needs assessment, connecting individuals with various resources, and providing basic employment skills for adults with Substance Use Disorder (SUD) through developing strong relationships with clients and community providers.
QUALIFICATIONS:
EDUCATION AND/OR EXPERIENCE REQUIREMENTS:
•Assoc. Degree in Addiction or related field + 3 years’ minimum experience working directly with individuals with substance use disorder (for Level I) or
Bachelor’s degree in behavioral science (Psychology, Social Work, Behavioral Sciences, or related field) required + minimum one (1) year of experience working directly with individuals with substance use disorder (for Level II)
•For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire
LICENSES AND CERTIFICATIONS:
•CADC-I required or CADC-R within 30 days of hiring and commitment to obtain with 1 year
•Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization’s auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations)
ESSENTIAL FUNCTIONS:
1. Works closely with other staff as a member of a treatment team;
2. Conducts comprehensive needs assessments for individuals;
3. Assists clients with accessing resources, benefits, and entitlement programs;
4. Obtains consents, releases of information, and requests collateral information from appropriate sources;
5. Develops treatment plans for substance-use disorders for assigned clients that is consistent with program and BCTS policies and procedures and applicable OAR's;
6. Conducts client Urine Analyses as needed and appropriate;
7. Follows all program policies and procedures for client case management and recordkeeping;
8. Responsible for recording information in the client files assigned to them and consistent with those policies and procedures and applicable OAR’s;
9. Provides support to SUD clients in achieving their treatment plan (ISSP/Individual Services & Support Plan) goals and objectives;
10. Develops and maintains strong collaborative relationships with a wide range of community partners, including (but not limited to): Self-Sufficiency and Seniors/People with Disabilities, the County Adult Community Justice, Public Health, Mosaic Medical and other primary care clinics including dental;
11. Stays abreast of community resources that individuals may be eligible for, including housing resources and housing options;
13. Receives training to become an OHP assister, to help individuals enroll in Oregon Health Plan if eligible;
15. Assist clients in determining if they are eligible for expungement of criminal record;
16. Facilitates psychoeducational and skills groups (if appropriate);
17. Maintains clinical documentation in a timely manner in client files in the EHR system and in compliance with BestCare timelines and regulatory standards;
18. Models appropriate counselor behavior to the newer staff, and helps train and orient new staff to the program;
19. In addition to being a positive role model for staff, also provides assistance to clients, demonstrating compassion and being effective in engaging clients in mental health treatment services wherever possible;
20. Other related duties as assigned.
PREFERRED:
•Bilingual in English/Spanish a plus
REQUIRED COMPETENCIES: Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:
• Demonstrated knowledge and ability to provide treatment of substance-related disorders including client evaluation, ASAM, and individual, group, family, and other counseling techniques; program policies and procedures for client case management and record keeping;
• Demonstrated knowledge of the philosophies, practices, policies, and outcomes of generally accepted and scientifically supported models of treatment, recovery, relapse prevention, and continuing care for addiction and other co-occurring problems;
• Advanced ability to develop and maintain strong, collaborative relationships;
• Ability to work effectively and respectfully in a diverse, multi-cultural environment is essential;
• Ability to electronically record information in client files in the EHR system timely and consistent with BestCare’s policies and procedures and applicable OAR’s;
• Proficiency in MS Office 365 (Word, Excel, Outlook), EHR platform, databases, virtual meeting platforms, internet, and ability to learn new or updated software;
• Strong interpersonal and customer service skills;
