Job Summary:
The Care Coordinator serves as the primary point of contact for care coordination needs of individuals seeking services at the ELC. The Care Coordinator assists individuals in navigating access to whole-person care (behavioral and physical), facilitates screening and assessment tools, provides referral and coordination for internal and external health needs, and helps to engage individuals in recommended care. The Care Coordinator works directly with individuals experiencing behavioral health challenges, as well as with individuals' supports, internal ELC team members and external community agency partners. This position reports to the Quality Assurance Director or their designee.
Major Job Responsibilities:
- Welcomes individuals to ELC through exceptional customer service, to help provide information about ELC, the services we offer and the community services responsive to their needs
- Assists individuals in smooth transitions to different types/levels of care
- Assists individuals in completion of required documentation
- Assists in completion of client health measures (e.g., blood pressure, height, weight)
- Responsible for data entry and monitoring through excel spreadsheets and EHR
- Monitors assigned CCBHC (Certified Community Behavioral Health Center) outcomes and coordinates with internal and external providers to increase frequency of desired outcomes
- Coordinates care among providers and provides necessary follow-up
- Documents activities in individual’s electronic health records
- Maintains confidentiality of all employee and client information
- Completes other duties as required
- Completes all required trainings as assigned
- Comply with ELC policies and procedures
Education and/or Experience:
- Graduation from High School or the equivalent, some college preferred
- Minimum 3 years clerical or general office experience/training required, medical office experience a plus
- Must possess a compassionate attitude and present a professional demeanor
- Familiarity with and/or lived experience with behavioral health challenges is preferred
