The Physical Therapist position is open in our Murrieta location and surrounding areas. This is a Per-Diem position during our day shift.
Requirements of the Physical Therapist:
· Graduate of a program of physical therapy approved by the APTA and the
· Council of Medical Education of the AMA.
· Current licensure for practice in the State of California.
· At least two year of experience in a clinical setting.
Physical Therapist Duties Include:
· Implements current physical therapy practice.
· Ensures quality and safe delivery of home health and hospice care services.
· Develops a Plan of Care based on the patient’s medical needs and discusses with the Case Manager and/or Clinical Manager. Revises plan as necessary for appropriate utilization of services and discusses changes with members of the Interdisciplinary Team.
· Promotes effective written/verbal communications.
· Uses equipment and supplies effectively and efficiently.
· Identifies need for intra-agency referrals as well as inter-agency referrals.
· Promotes education for clients, client families and the community.
· Participates in personal professional growth and development.
· Conferences with Case Manager and/or team members during the patient’s course of treatment.
· Provides inservice to other staff members as requested.
