Job Title:  Physical Therapist 

Hours: 30-40 per week;  Generally  M-F, 8:45 a.m. - 5:00 p.m. (flexible schedule work: Sunday - Thursday or Tuesday - Saturday); weekends if necessary


We are seeking a dedicated and compassionate full-time Physical Therapist to join our team. This role offers an excellent opportunity to make a meaningful difference in patients' lives by helping them regain mobility, reduce pain, and improve their overall quality of life through personalized rehabilitative care.



Essential Job Duties: 

  • Restores patient’s function, alleviates pain and prevents disabilities by planning and administering medically prescribed physical therapy. 
  • Provides quality care by assessing and interpreting evaluations and test results. 
  • Develops a treatment plan for each patient based on initial and ongoing evaluations, including short- and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement. 
  • Determines physical therapy treatment plans in consultation with physicians or by prescription. 
  • Implements Plan of Treatment on a consistent schedule and adjusts this schedule and plan according to patient progress. 
  • Helps patient accomplish treatment plan by administering manual exercises, massage, and using equipment as necessary. 
  • Evaluates mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable. 
  • Evaluate and record patient progress, modify treatment plans, and try new treatments if required. 
  • Evaluate the use and fit of prosthetic and orthotic devices by observing, noting patient’s progress, and recommending adjustments and modifications. 
  • Documents patient care services according to approved medical documentation and facility documentation procedures. 
  • Reports patient progress to the patient, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients. 
  • Develops a plan for patients following completion of their Plan of Treatment with appropriate in-home care services. 
  • Leads by example and exhibits professionalism at all times. 
  • Holds themselves accountable and encourage collaboration and flexibility within the department and interdepartmental teamwork for positive outcomes. 


Requirements

Qualifications:

  • Education: Graduate of an accredited therapy program
  • Experience: 2-3 years of related experience; or an equivalent combination of education and experience 
  • Licenses/Certifications: Current Physical Therapist state license; CPR certification
  • Excellent verbal & written communication
  • Time management
  • Sound judgment & decision making 
  • Active listening
  • Ability to work effectively as part of a multidisciplinary team
  • Compassionate, patient-focused approach to care
  • Previous experience in a healthcare setting, preferably with geriatric populations is a plus
