Description
The Rehabilitation Counselor (RC) is responsible for organizing services and support to facilitate a client’s recovery and achievement of life goals to improve functioning in the home, school or workplace, and community.
Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Meet with clients regularly and consistently to work on client goals (six times a month for adults, four times a month for minors).
- Develop and implement Individual Rehabilitation Plans (IRPs) with clients to outline goals for recovery.
- Conduct regular in-home or community based client visits to provide individualized case management, counseling, coaching, and skills training as needed.
- Link clients to government and community resources as needed, and assist in clients obtaining those resources.
- Ensure client maintenance of physical and mental health and well-bieng.
- Support client cooperation with treatment recommendations from other providers (therapy, medication management, medical appointments, etc.), and maintain communication with those providers.
- Provide and/or coordinate transportation to and from scheduled appointments.
- Complete all documentation including but not limited to, notes, monthly progress summaries, and quarterly IRP reviews, in a timely and accurate fashion.
- Report regularly to the immediate supervisor, and engage in supervision and training.
- Ensure compliance with all agency policies and procedures and state regulations governing the provision of Psychiatric Rehabilitation Services.
- Communicate regularly and consistently with staff members and its affiliate organizations in regards to scheduling, documentation, and client care.
- Maintain scheduled client appointments based on agreed-upon schedule.
- Assist with mental health emergencies as needed.
- Be a positive representative of the organization by upholding standard messaging and its brand to clients, internal and external providers, referring partners, community affiliates, and any other external third party.
- Attend scheduled meetings and trainings on-time as assigned; or make efforts to communicate any tardiness or absenteeism. Such meetings include (but are not limited to):
- Company-wide staff meetings
- Assigned trainings or forums
- Clinical Supervisions
- Check-ins with supervisors and management
- Appointments with clients, providers, or external partners
QUALIFICATIONS:
- Ability to strategically, logically, and realistically analyze, evaluate and develop solutions for client’s rehabilitation needs.
- Excellent interpersonal skills with the ability to effectively build relationships with individuals of all ages, ethnicity, education, and work experience.
- Understands and maintains confidentiality and all regulations regarding HIPAA and 42 CFR Part 2.
- Exhibits professionalism and a friendly demeanor, and is a positive representative of the company over the phone and in person
- Solid written and verbal communication skills.
- Comfort with computers and technology as needed to input clinical documentation.
- Strong organizational skills to manage multiple clients and scheduling.
- Reliable, ethical, and honest work ethic.
