Licensed Practical Nurse
Description

Provide patient care in home, and assisted living environments.

Key Responsibilities:

  •  Provide wound care, including cleaning, dressing, and monitoring for infection.
  •  Teach patients and their families how to manage care, administer medications, and perform necessary health maintenance in the home.
  •  Collaborate with physicians, therapists, and other members of the care team. Report changes in patient condition to the appropriate personnel.
  •  Maintain accurate and complete documentation of patient assessments, treatments, and progress.
  •  Assess the home environment for safety and make recommendations for improvement.

Qualifications:

  •  Completion of an accredited LPN/LVN program, and a current valid state LPN or LVN license.
  •  Clinical skills and experience
  •  Critical thinking and problem-solving skills
  •  Excellent communication skills to interact with patients, families, and colleagues effectively.
  •  Proficiency in various wound care techniques, products, and healing processes.
  •  Experience in PICC lines, midlines, and peripheral IV care and management.
  •  Ability to work collaboratively with a multidisciplinary healthcare team.
  •  Organizational and time-management abilities.
  •  Physical stamina, as the job can involve lifting and standing for long periods.
