- Manage Data: Accurately record all client demographics and referrals in the database in accordance with federal requirements.
- Collaborate & Advocate: Consult with staff across all departments and programs to provide holistic support and develop informal support networks within the community.
- Professional Growth: Once eligible, complete all training requirements for the InformUSA CRS-A/D certification.
Experience: At least 1 year of direct, hands-on experience in case management, social services, care coordination, or a related non-profit role.
- Education: High School Diploma or GED required; Obtain InformUSA CRS-A/D Certification (paid by the agency).
- Communication Skills: Requires strong written and verbal communication skills in order to write accurate progress notes and present information clearly to the public.
- Tech Savvy: Ability to use Microsoft and Google programs, operate a multi-line phone system/headset, and learn the agency’s data system.
- Physical/Travel: Must have a valid driver's license and proof of vehicle insurance, along with reliable transportation in order to meet job requirements.
- Personal Attributes: A positive first impression, the ability to maintain ethical social work boundaries, and a desire to work both independently and collaboratively.
